Household POC Information:
Each participant (or parent/guardian for minors) must read the below liability waiver. Check the box and type initials in the registrations section to confirm acceptance of the waiver.
1. LifePointe WISC Takeover Participation Waiver: Every player (or their parent or guardian, if the player is under age 18) must read and sign this waiver form to participate in this event. Signatures on the registration form signify each person has read, understands, and abides by this information.
2. Release Of Liability: There are risks connected with my (or my child’s) participation in this event and its related activities. I release and discharge LifePointe Christian Church, event volunteers, church staff, elders, and directors from all actions, suits, and demands whatsoever in law or in equity, including but not limited to the risk of personal injury from playing in the event and the risk of loss of personal property by theft or otherwise.
3. Release Of Photo/Video Rights: Further, I hereby grant full permission for event organizers to photograph or video record any or all of my (or my child’s) participation in this event, which may be used for publicity, promotions, without any reimbursement of any kind due to me, or the need to pay me any fee.
